His Imperial Majesty Emperor Isaac IV was the second son of Emperor Manuel V. He came to the throne after the poisoning of his brother, Emperor Manuel VI. Until his twenty-first birthday, Pantocratoria was ruled over by a regency council jointly appointed by the Empress Dowager Isabelle d'Amboise and the Imperial Parliament. On his twenty-first birthday, he married Helen d'Isakiosopolis, and assumed the full powers of his office.

In 1744, a native uprising in the Exarchate of New Jerusalem required him to come to the Exarch's aid with the Varangian Guard and his other legions. The Forties Uprisings, as the series of the revolts came to be known, kept the Emperor's armies busy for years, and Isaac IV's life was cut short at the age of 30 by malaria, dying on campaign.
